When do I receive my ID Card If you enroll by credit card online, you will
Receive your ID Card within 5 days.  If you Choose the enroll by phone, you will immediately covered and cards will be mailed out same day.
Choosing a provider Once you join you will receive a listing of Participating Providers to choose from. We have 100's of providers to choose from. When you visit their office, just show them your ID card for the savings.

Single  Plan   $99.00 Individual enrollment for the entire year

2 Person Rate $149.00 a 2 person family enrollment for the entire year

Family Plan     $199.00 Family of 3 or more enrollment savings for the entire year

Monthly Payment: Make monthly payments by Automatic Bank Draft.
Just email us at hpmn@aol.com and we will mail out the form to enroll.
The HealthPlus Network will automatically withdraw from your checking
